At Trinity House, we are aware further assessments of elements of the service user’s needs are required, to ensure that the care we are providing is relevant to helping the resident achieve their full potential. Every service user has access to their plan and is encouraged to participate as fully as possible in the care planning process. This ensures that the best possible long-term outcome is achieved for each service user and if there are signs of relapse we are to revisit these areas.
The areas covered in the tailored action/care plan include: –
- • Monitoring mental health
• Monitoring medication – side effects
• Help in managing finances, budgeting and benefit claims
• Support with shopping requirements
• Support with cooking skills and daily living skills
• Guidance and support with personal hygiene and appearance
• Developing social skills & maintaining positive community presence
• Guidance and support with personal hygiene and appearance
• Help in setting up and maintaining tenancy
• Support to acquire education and therapeutic employment
• Encouragement of positive social and family contact
• Support to recover from addictions and safe use of alcohol/substances.
Regular activity & structure addressed daily and as part of care plan:
- • getting up on time and tidying up their room – laundry, ironing and storing clothes
• attending to their personal hygiene
• ensuring they are suitable groomed /dressed for the day.
• preparing meals
• providing support with engaging with various agencies – CMHT’s, GP, local Jobcentre, Benefits agencies etc.
• Management of finances – handling cash, banking and budgeting
• Shopping – providing advice and escort as required and encouraging independence and choice
• Participation in communal living activities – cooking and washing-up
• Verbal and written skills – ensuring residents are encouraged in learning in the community through in-house use of computer training and attending local college.
